Hand in Hand Care is the most reliable and trustworthy Geriatric Care Management service in Sarasota County. We are true "hometown nurses" that specialize in working with seniors and their caregivers.
We realized that in our community, there are many caregivers who could use some assurance, a back-up person, and someone to lean on.

$150 One-Time FeeMonthly Service Tier
Following the initial assessment and consultation, you will be able to schedule a monthly in-person visit. You will also have unlimited communications via phone, text, or email, thus providing for ongoing updates and continued evaluation.
$150 One-Time + $125/month
