Hand in Hand Care is the most reliable and trustworthy Geriatric Care Management service in Sarasota County. We are true “hometown nurses” who specialize in working with seniors and their caregivers, with particular expertise in dementia care and comprehensive support services for individuals and families.
Registered Health Information
Our Care Managers are also Registered Nurses! With our Caregiver Assurance Program, we offer that essential support, reducing the risk of burnout or self-neglect.

Essential Assurance
1st Tier — Hand in Hand Care Securely Stores Information
A Hand in Hand Care Manager will complete a comprehensive assessment and gather your health history, medical information, and important planning documents. This information is securely maintained, allowing our team to respond quickly and efficiently should care management services become necessary.
Our team will help determine an appropriate emergency back-up plan and will check in with you quarterly (by phone or email) to assess the need for any updates or changes to your personalized plan.

Premier Assurance
2nd Tier — Hand in Hand Care Monthly Visit Service
Our team will securely retain the health information that you provided during our time together (as outlined in 1st Tier Option).
Following the initial $250 assessment and consultation, we will be available for one in-person or remote visit each month to provide support and advocacy services.
This option also includes unlimited access throughout the month via phone, text, or email should you have questions, need guidance, want updates, or simply talk through a situation.

3rd Tier — Hand in Hand Care Concierge Care Management
The Platinum Advocate plan includes all of the benefits of the first two tiers while providing the highest level of personalized care management and advocacy. This concierge-level program is designed for clients who require ongoing oversight, complex care coordination, and comprehensive support.
Following the initial assessment and consultation, your dedicated Care Manager will provide bi-weekly in-person clinical and social visits to monitor health, safety, and overall well-being.
During these visits we are available to coordinate communication between healthcare providers and family members, and assist with medication management and follow-up care.
Platinum Advocate clients receive priority 24/7 access to our care management team for urgent situations, along with comprehensive coordination of home care providers, therapists, attorneys, and other professionals involved in your care.
This service provides proactive oversight and expert advocacy, giving clients and their families the confidence that every aspect of care is being managed with compassion, professionalism, and attention to detail.
